Abhishek Sharma’s dismissal triggered a collapse in which India lost three wickets for nine runs.
A mix-up between Tilak Varma and Axar Patel cost India a run out in the 17th over.
Shamar Springer and Shamar Joseph took two wickets each for West Indies.
West Indies, chasing 224, finished on 209 for 7.
Shimron Hetmyer made 43 off 26 balls and Kamil Pooran made 42 off 27 balls.
Shamar Springer remained not out on 35 off 11 balls, and Sherfane Rutherford made 32 off 20 balls.
Arshdeep Singh took three wickets, including Rutherford, and dismissed Shai Hope for 5 in the powerplay.
Naman Dhir took two wickets, including Hetmyer and Pooran.
Arshdeep’s double strike in the 18th over swung the match towards India, according to Outlook.
Hetmyer was dropped in the powerplay and was later dismissed by Dhir.
Mayank Yadav went wicketless and conceded 34 runs in four overs.
The series stands at 2-1 in India’s favour, after West Indies won the second T20I in Ranchi by six wickets.
India won the first T20I in Lucknow by eight wickets.
